AHCA Accidental Death & Dismemberment Insurance

Included with your company’s membership is one $20,000 accidental death and dismemberment policy. This policy is normally written in the name of the company's owner but can be changed upon request.

You may extend this benefit to all your employees/drivers at an additional cost. You may also upgrade this policy to $50,000 or $100,000 at an additional cost. Voluntary policies for children and spouses are also available at a very low rate. See the AD&D Brochure for details.

In case of a loss, monies are paid according to the specifications that are indicated on the back page of the Certificate of Insurance, which is enclosed in your membership package. If you wish to designate a beneficiary, you must complete the Beneficiary Designation Form and return it to our office.
